To Our New Patients
If
you are a new patient and have scheduled an appointment, there are a
few things you should bring to the office:

If you have had any
chest
x-rays and/or CT scans, the doctor will need you to bring the actual
films. Contact the same place you had them taken to arrange
picking them up. You can also request that the scans or
x-rays be put on a CD for the physician to view.

A list of any
medications
that you may be taking now. List the dosage of the medication
and how often it is taken.

Any other information
pertaining to your lungs, such as x-ray/CT scan reports or pulmonary
function test results.

If
your insurance requires a referral, you must bring this with you to our
office. Contact your
primary physician to obtain one.

Bring your insurance
cards
so we may make a copy for your chart.

You must present a photo ID at the
time of your appointment.

links, print the forms and have them completed when you come for
your appointment.
